Community Health Assessment (CHA)
To better understand the evolving health needs of our residents, JCPH conducts a Community Health Assessment (CHA) every five years. The 2023 CHA process included surveying 1000+ county households, analyzing local and state health data, and examining the potential health impact of local policies. We also worked closely with community focus groups and key stakeholders to better understand where our residents would like to see the most change. In the video below, we explain this process in more detail and unpack the two key priority areas identified during its creation.
We publish the CHA as a data resource for local leaders, students, and community members who are working for the betterment of Eastern Jackson County. In the 2023 CHA, you’ll find information about our county’s mental health, communicable diseases, maternal & child health, environmental health risks, and more.
Community Health Improvement Plan (CHIP)
Our Community Health Improvement Plan (CHIP) takes what we learned in the CHA process and translates it into action! This multi-year plan addresses residents’ top health concerns through our work at JCPH, keeping our programs relevant and helpful for our community.
