Community Health Assessments & Improvement Plans
Just as a person’s health needs change throughout their life, our county’s health is always evolving! Our Community Health Assessment (CHA) combines resident input and research to provide a detailed overview of our county’s health. It provides an in-depth look at the health impact of various topics, such as mental health and economic stability.
You can find a video presentation summarizing the CHA, including key statistics and takeaways, here on our CHA page.
Our Community Health Improvement Plan (CHIP) takes what we learned in the CHA process and translates it into action! This multi-year plan addresses residents’ top health concerns through our work at JCPH, keeping our programs relevant and helpful for our community.
