Birth & Death Certificates

To pick up a birth or death certificate, fill out our online form or walk-in during business hours.

Pick-up hours are 9 AM to 4 PM. Please bring a photo ID (ex: driver’s license) or two pieces of mail with your name and current address (ex: an electric bill and water bill).

Physical (printed) documents are required for identification.
